California Medicaid exclusion list

ExclusionHQ can't check the Medi-Cal Suspended and Ineligible Provider List automatically yet: the open-data file refused our download; being worked on. Check it yourself on the state's page and keep a record.

The free check below still covers the federal lists every California provider needs: the OIG LEIE and SAM.gov.

Checking the California list yourself

Download the current list from the state's page in your browser, search it for each person's surname, and keep the file with the date you checked it.

Check before each person starts and then on the schedule your Medicaid provider agreement sets. For the federal lists, OIG says checking every month best limits the risk, because it updates its list monthly (see monthly exclusion screening).

California on the federal lists

Of the people on the OIG exclusion list (the LEIE copy of Nov 6, 2026), 9,435 had a California address when they were excluded, 163 of them in the last 12 months. 47% were excluded on the mandatory grounds (convictions for program fraud, patient abuse or neglect, felony health care fraud or felony controlled-substance offences) and 45% after a health care licence was revoked, suspended or surrendered.

The most common professions among them: nurses and nurse aides (3,088), chiropractors (623) and general practice physicians (435).

SAM.gov lists 5,978 excluded individuals with a California address, many of them the same people.

An OIG exclusion covers every federal health care program, in every state, so someone excluded elsewhere who moves to California is still excluded here: check every hire against the federal lists, wherever they come from.

Neighboring states

Someone who has worked across the state line can be on that state's Medicaid list without being on California's. If you hire from neighboring states, check their lists too:
