Arizona Medicaid exclusion list
ExclusionHQ can't check the AHCCCS provider suspensions and terminations automatically yet: published as a PDF only. Check it yourself on the state's page and keep a record.
The free check below still covers the federal lists every Arizona provider needs: the OIG LEIE and SAM.gov.
Add dates of birth: the LEIE lists one for almost everyone, and it is what tells two people with the same name apart.
Checking the Arizona list yourself
Open the latest PDF on the state's page, search it for each person's surname (Ctrl+F, or ⌘F on a Mac), and save a copy of the file with the date you checked it.
Check before each person starts and then on the schedule your Medicaid provider agreement sets. For the federal lists, OIG says checking every month best limits the risk, because it updates its list monthly (see monthly exclusion screening).

Arizona on the federal lists
Of the people on the OIG exclusion list (the LEIE copy of Nov 6, 2026), 2,644 had an Arizona address when they were excluded, 52 of them in the last 12 months. 31% were excluded on the mandatory grounds (convictions for program fraud, patient abuse or neglect, felony health care fraud or felony controlled-substance offences) and 66% after a health care licence was revoked, suspended or surrendered.
The most common professions among them: nurses and nurse aides (1,773), personal care providers (40) and therapists (40).
SAM.gov lists 1,059 excluded individuals with an Arizona address, many of them the same people.
An OIG exclusion covers every federal health care program, in every state, so someone excluded elsewhere who moves to Arizona is still excluded here: check every hire against the federal lists, wherever they come from.
Neighboring states
Someone who has worked across the state line can be on that state's Medicaid list without being on Arizona's. If you hire from neighboring states, check their lists too:
