New York Medicaid exclusion list (OMIG)
The NYS Medicaid Exclusion List is published by the NY Office of the Medicaid Inspector General (OMIG). ExclusionHQ downloads it every day it changes and checks plan rosters in New York against it, alongside the OIG LEIE and SAM.gov.
The free check below covers the federal lists; plans add this state's list. You can also search it on the state's own page.

Add dates of birth: the LEIE lists one for almost everyone, and it is what tells two people with the same name apart.
About the New York list
Each entry gives the name, NPI, licence number, provider type and effective date.
It has no dates of birth, so names must agree exactly or as a nickname and its full form, and NPIs and licence numbers match on their own. A name match there is a result to verify against the entry, not a confirmed match. See how matching works.
If a download fails, or the number of people on it falls by more than a fifth, the last good copy stays in use and the coverage page says so.
New York on the federal lists
Of the people on the OIG exclusion list (the LEIE copy of Nov 6, 2026), 4,038 had a New York address when they were excluded, 71 of them in the last 12 months. 62% were excluded on the mandatory grounds (convictions for program fraud, patient abuse or neglect, felony health care fraud or felony controlled-substance offences) and 28% after a health care licence was revoked, suspended or surrendered.
The most common professions among them: nurses and nurse aides (1,295), general practice physicians (339) and owners and operators (234).
SAM.gov lists 2,913 excluded individuals with a New York address, many of them the same people.
An OIG exclusion covers every federal health care program, in every state, so someone excluded elsewhere who moves to New York is still excluded here: check every hire against the federal lists, wherever they come from.
Neighboring states
Someone who has worked across the state line can be on that state's Medicaid list without being on New York's. If you hire from neighboring states, check their lists too:
