ExclusionHQ

Claude connector

ExclusionHQ connects to Claude as a remote MCP connector, so you can check named people against the federal healthcare exclusion lists from a conversation. Nothing is installed on your computer. It reads the published lists and returns text; it can't change anything in your account.

Set up

Connector address: https://exclusionhq.com/api/mcp. You sign in once with an email address or a Google account. If you're new, that creates a free ExclusionHQ account.

  • Claude on the web, desktop or mobile: add it in one click, or go to Customize, then Connectors, then Add custom connector.
  • Claude Code: run claude mcp add --transport http exclusionhq https://exclusionhq.com/api/mcp, then /mcp to sign in.

Tools

All three are read-only.

  • check_exclusions (people): Checks up to 50 people against the OIG LEIE and SAM.gov exclusions. Each person comes back as a match, to verify, same name but a different person, or not on the lists, with the date of each list copy.
  • list_coverage (state (optional)): Shows which lists we hold and how current each copy is. Given a state, says whether it has its own Medicaid exclusion list, links to the official page, and says why if the list has to be checked by hand.
  • whoami (none): Shows which account is connected and how many checks it has used today.

Example prompts

  • Check these new hires against the OIG and SAM exclusion lists: Ada Quillfeather, born 04/12/1986; Remy Ortolan, born 1979-09-30; Juno Pemberley.
  • Is this contractor excluded from federal health care programs? Remy Ortolan, date of birth 1979-09-30.
  • Does New York have its own Medicaid exclusion list, and how recent is your copy of it?
  • How current are the OIG and SAM lists you check against?
  • Which account is ExclusionHQ connected to, and how many checks are left today?

What a result means

A result is a potential match, not a determination. Before anyone acts on one, a person has to verify it on the agency's own record. Do not make a hiring or employment decision from a result alone. Results are not legal advice.

  • Match: the name and a date of birth or ID agree with a listed person.
  • Verify: the name matches, but the list can't settle whether it's the same person. SAM.gov publishes no dates of birth, so name matches there often land here.
  • Same name, different person: someone with this name is listed, and the record shows it's someone else. That person's details aren't shown.
  • Not on the lists: no match in the list copies dated in the result.

How matching works has the method and what no name check can catch.

Data and limits

  • The connector searches two federal lists: the HHS-OIG List of Excluded Individuals/Entities (LEIE) and SAM.gov exclusions. See every list and its date.
  • It doesn't search state Medicaid lists, licences or criminal records. State lists are checked for rosters in an ExclusionHQ account.
  • Up to 50 people per check and 30 checks a day per account. The count resets at midnight UTC.

Privacy

The connector receives only what Claude sends to a tool: the names and dates of birth you ask it to check. They're used for that one request and aren't stored. Your account keeps a count of today's checks. The connector doesn't read your conversations, memory or files. Results go back into your Claude conversation. Don't send Social Security numbers or patient information. Full details are in the privacy policy.

Troubleshooting

  • “Today's limit of 30 checks”: the daily limit per account. It resets at midnight UTC.
  • “Couldn't read the date of birth”: give it as YYYY-MM-DD or MM/DD/YYYY. Until then, that person is checked by name only.
  • “The exclusion lists aren't available”: nothing was checked. Try again in a few minutes.
  • Sign-in fails or the tools don't appear: disconnect the connector in Claude's settings, then connect it again.
  • Anything else: support@wikivec.com.
