Minnesota Medicaid exclusion list

ExclusionHQ can't check the Minnesota DHS excluded providers automatically yet: the file is behind a CAPTCHA. Check it yourself and keep a record.

The free check below still covers the federal lists every Minnesota provider needs: the OIG LEIE and SAM.gov.

Checking the Minnesota list yourself

Download the current list from the state's page in your browser, search it for each person's surname, and keep the file with the date you checked it.

Check before each person starts and then on the schedule your Medicaid provider agreement sets. For the federal lists, OIG says checking every month best limits the risk, because it updates its list monthly (see monthly exclusion screening).

Minnesota on the federal lists

Of the people on the OIG exclusion list (the LEIE copy of Nov 6, 2026), 1,381 had a Minnesota address when they were excluded, 84 of them in the last 12 months. 59% were excluded on the mandatory grounds (convictions for program fraud, patient abuse or neglect, felony health care fraud or felony controlled-substance offences) and 33% after a health care licence was revoked, suspended or surrendered.

The most common professions among them: nurses and nurse aides (427), health care aides (199) and personal care providers (169).

SAM.gov lists 792 excluded individuals with a Minnesota address, many of them the same people.

An OIG exclusion covers every federal health care program, in every state, so someone excluded elsewhere who moves to Minnesota is still excluded here: check every hire against the federal lists, wherever they come from.

Neighboring states

Someone who has worked across the state line can be on that state's Medicaid list without being on Minnesota's. If you hire from neighboring states, check their lists too:

Monthly monitoring

Monitor your Minnesota roster

Save your roster once. ExclusionHQ re-checks everyone against the OIG LEIE, SAM.gov whenever one updates, emails you when something changes, and keeps a dated audit report for every check. The Minnesota DHS excluded providers isn't checked automatically yet. From $39 a month for up to 100 people.
